NHS: Getting a dementia diagnosis
If you're worried about your memory, or think you may have dementia, it's a good idea to see a GP. If you're worried about someone else's memory problems, encourage them to make an appointment with a GP and perhaps suggest that you go with them. Getting a diagnosis gives you and your family the best chance to prepare for the future.

Dementia Assessment and Support Service (DASS)
Providing a dementia care pathway covering Shropshire and Telford & Wrekin, working closely with other agencies and services throughout the county who may be able to offer additional support and advice to you and your family. Initial assessment – diagnosis – post-diagnostic support – treatment – interventions and person-centred support – signposting to external services.

Notifying the DVLA
If you have a driving licence you must tell DVLA if you have a diagnosis of dementia, using the form CG1 on the website. The DVLA will take any decision about a person’s ability to continue to drive based on a doctor's report. Many people are able to continue to drive after a diagnosis.
